Forms needed to request information or services from the State Records
Center and Archives may be downloaded in either of three formats, MS
Word, MS Excel, and Adobe Acrobat (pdf version).
Records Management Division
Records Management makes their forms available only in a PDF format. The forms are text ready for data entry and printing. They cannot be saved with text if using the free version of Adobe Acrobat Reader. To save the forms with the text, the full version of Adobe Acrobat must be used.
